Cortisol is one of the most important stress hormones
Due to the numerous neuroendocrine interactions of cortisol, an excess of cortisol can lead to metabolic disorders with obesity, visceral fat distribution and increased blood lipid levels, diabetes, immunodeficiencies and a reduced defence against infections and tumours. This results from the inhibition of the cellular immune response with a simultaneous relative strengthening of the humoral immune response. It contributes to increased secretion in acute and repeated stress situations. The concentration in the body is highest between 7 and 8 o'clock in the morning.
Due to the time-consuming nature of a daily cortisol profile for the patient, numerous studies have looked for a measurement method that allows samples to be obtained more easily and therefore allows the test to be carried out more cost-effectively. These requirements were realised with the "Cortisol Awakening Response" (CAR).
The Cortisol Awakening Response (CAR) is used to assess the adrenal activity of the hypothalamic-pituitary-adrenal axis (HHNA). Within the first hour after getting up in the morning, the patient takes five saliva samples for cortisol determination.
O. W. Schweisthal (2007) analysed both measurement methodological aspects and the influence of various variables on the results of the CAR in a quantitative meta-analysis. The information content of the CAR is specified by calculating various static characteristic values. The AUCI-MAX (Aurea Under Curve), for example, is a measure of adrenal reactivity to the awakening stimulus. It comprises the absolute increase, the time span from awakening to reaching the maximum value and the personal time profile (variations in the slope of the increase over time).
