Zonulin is a marker of potentially increased permeability of the intestinal epithelial cells and may indicate the presence of disease

If the gut is colonised by an abnormal mix of microorganisms, the gut flora can only fulfil its physiological functions to a limited extent. The resulting disruption of the microbial barrier ultimately leads to pathological mechanisms that cause latent inflammatory changes in the mucosa and subsequent impairments in permeability. This makes it easier for pathogenic bacteria, viruses, fungi or parasites, in the context of microecological disturbances, to penetrate mucosal receptors more readily, multiply more rapidly and trigger infections.

Depending on the species of micro-organism, microbial metabolism ultimately produces either substrates that are beneficial to the intestinal environment or those that harm the system in various ways. A loosening of the ‘tight junctions’ in the intestinal epithelium leads to increased permeability of the mucosa, a condition known as leaky gut. This allows many molecules to pass from the gastrointestinal tract into the bloodstream and trigger various reactions.

Endotoxin is of particular significance. This molecule, also known as lipopolysaccharide (LPS), originates from the surface of Gram-negative bacteria and acts as an inflammatory marker. Chronic overloading of the blood with LPS can lead to what is known as ‘silent inflammation’. This persistent inflammatory state can contribute to further conditions such as metabolic syndrome, obesity, type 2 diabetes or atherosclerosis. The LPS concentration in the blood can be measured directly using laboratory diagnostic methods.

Zonulin, a protein, binds to specific receptors on the epithelial cells of the intestinal wall. This triggers a series of biochemical processes that lead to the opening of tight junctions and increased permeability of the intestinal epithelial cells. In patients with active coeliac disease, levels of zonulin and zonulin antibodies are elevated. Furthermore, many coeliac patients are affected by other autoimmune diseases, such as insulin-dependent diabetes, multiple sclerosis and rheumatoid arthritis. It is believed that elevated zonulin levels play a decisive role in this context.
