Zonulin is a marker of a potentially increased permeability of the intestinal epithelial cells and can provide indications of disease

If the gut is improperly colonised, the gut flora can only fulfil its physiological functions to a limited extent. The resulting disturbance of the microbial barrier ultimately leads to pathomechanisms that cause latent inflammatory changes of the mucosa and subsequent permeability disturbances. This makes it easier for pathogenic bacteria, viruses, fungi or parasites, in the context of microecological imbalances, to penetrate mucosal receptors, multiply more rapidly and trigger infections.

Depending on the species of the microbe, microbial metabolism ultimately produces either substrates that are beneficial for the intestinal environment or ones that harm the system in various ways. A loosening of the "tight junctions" in the intestinal epithelium leads to an increased permeability of the mucosa, which is known as leaky gut. Thus many molecules can cross from the gastrointestinal tract into the bloodstream and elicit various reactions.

Endotoxin plays a special role. This molecule, also called lipopolysaccharide (LPS), comes from the surface of gram-negative bacteria and acts as an inflammatory marker. A chronic overload of the blood with LPS can lead to a so-called 'silent inflammation'. This constant inflammatory state can promote further conditions such as metabolic syndrome, obesity, type 2 diabetes or atherosclerosis. The LPS concentration in the blood can be determined directly using laboratory diagnostic methods.

Zonulin, a protein, binds to specific receptors on the intestinal epithelial cells. This triggers a series of biochemical processes that lead to the opening of the tight junctions and an increased permeability of the intestinal epithelial cells. In patients with active coeliac disease, levels of zonulin and zonulin antibodies are elevated. Furthermore, many coeliac patients are affected by other autoimmune diseases, such as insulin-dependent diabetes, multiple sclerosis and rheumatoid arthritis. It is believed that elevated zonulin levels are a decisive influencing factor in this.
